The Clinical Impact of Floaters and Success of Laser Vitreolysis

چکیده

It is well known that the vitreous forms 80% of the ocular body and consists of water (98%) and a network of collagen fibrils and hyaluronic acid (2%). Thus, the vitreous is typically characterised as a transparent gel firmly attached to the retina. Vitreal transparency is key as it allows transmission of light through unobstructed media to the retina for visual stimulation. With age, the vitreous begins to liquefy due to molecular rearrangement between hyaluronic acid and collagen macromolecules. This involves cross-linking and aggregation of collagen causing light scattering and consequently “floaters” and entopticlike symptoms. Simultaneously, there is a weakening of the vitreous and retinal interface causing separation of the posterior vitreous cortex, a phenomenon known as posterior vitreous detachment (PVD). During this process, the acute onset of multiple floaters and those of longstanding appearance causes functional visual problems for patients and bothersome symptoms1. IMPACT OF FLOATERS Many physicians will typically rule out events of retinal detachment or vitreous haemorrhage before discharging a patient with symptomatic floaters. However, the implication of floaters is misunderstood and under appreciated.

Cataracts induced by neodymium-yttrium-aluminium-garnet laser lysis of vitreous floaters.

BACKGROUND Neodymium-yttrium-aluminium-garnet (Nd:YAG) laser vitreolysis has been proposed as a treatment modality for symptomatic vitreous floaters. The purpose of this paper is to report two cases of cataracts associated with posterior capsular compromise, induced by Nd:YAG laser vitreolysis for symptomatic vitreous floaters. METHOD Case series. RESULTS Two patients who underwent ND:YAG l...

Eligibility criteria for Nd-YAG laser treatment of highly symptomatic vitreous floaters.

Ten eyes of nine patients were treated for very disturbing vitreous floaters with the technique of Nd-YAG laser vitreolysis. The Scanning Laser Ophthalmoscope (SLO) was used to objectivate the position, the size and the motility of the vitreous floaters with respect to the patient's visual axis, which can be precisely located with the SLO. With this technique it was possible to define more prec...

YAG Laser Vitreolysis for the Treatment of Floaters

Although notoriously bothersome, floaters typically resolve within weeks or months of appearing. In some cases, however, they persist for much longer.2 When floaters persist or are present at disabling levels; removal of the vitreous humor (vitrectomy) is considered a viable treatment option. However, it is a highly invasive procedure that carries a significant risk of complications, such as in...

Floaters and reduced contrast sensitivity after successful pharmacologic vitreolysis with ocriplasmin

Purpose To describe the onset of floaters and reduction in contrast sensitivity (CS) following successful pharmacologic vitreolysis with ocriplasmin for the treatment of vitreo-macular traction (VMT) in a patient with previously normal CS. Observations A 65-year-old woman with a past ocular history of normal visual acuity (VA = 20/26) and contrast sensitivity (1.81% Weber) presents with a 4-m...

Rapid Cataract Progression after Nd:YAG Vitreolysis for Vitreous Floaters: A Case Report and Literature Review

PURPOSE We report a case of rapid cataract progression after Nd:YAG vitreolysis for vitreous floaters. CASE REPORT A 55-year-old man presented with acute onset of blurred vision following Nd:YAG vitreolysis for symptomatic floaters in the left eye. His initial best corrected visual acuity (BCVA) was 20/1,000 in the left eye. Ocular examinations showed frost-like opacities of the lens and a su...
